  • Guidance in reviewing Advance Directives, POLST, and other medical documents and ensuring they are shared with loved ones and care team; referrals to professionals if needed. 

  • Creation of a system for storing essential documents, passwords, and online login details, and a plan for sharing those with appropriate parties, when needed. 

  • Assessment of household needs with coordination or referrals for services such as cleaning, home maintenance, organization, and donation of personal belongings. Provide wellness check- ins and report concerns with family and care team; help organize medications and set up reminders; run errands such as groceries or pharmacy visits to reduce stress and maintain independence.  

  • Assistance with setting up online bill payments and streamlining financial responsibilities. 

  • Life review, journaling, photo archiving, and video interviews to preserve personal stories and memories. 

  • Education on available community resources and support in locating trusted in-home caregivers. 

  • Compassionate presence, companionship, and a listening ear for clients and families; Grief coaching.

  • Facilitated discussions around death care wishes, planning, and emotional readiness. Collaborate on planning a Celebration of Life—whether held before or after death—that reflects your personality, values, and unique story. Make sure your wishes are conveyed to family/loved ones. 

Fee Structure

The first appointment (up to an hour) is complimentary and serves to determine the client’s needs and if those needs are the appropriate fit with The Last Chapter

Hourly Support

For those who need occasional check-ins, guidance, or help with a specific task like planning a vigil, assistance writing a legacy letter, or organizing documents.

Companion Care

Includes 6 hours of emotional, logistical, or wellness support over a flexible timeframe. Ideal for clients seeking consistent guidance.

Cleaning & Organization

6–8 hours of supportive, judgment-free help with sorting, simplifying, and organizing possessions and/or documents. Together, we’ll clarify what to keep, gift, donate, or let go of, and create peace of mind by lessening the burden on your loved ones.

Sliding Scale & Accessibility

I believe end-of-life support should be available to everyone. If cost is a barrier, please don’t hesitate to reach out. Sliding scale options or payment plans may be available depending on your situation.


What I Don’t Offer

As an end of life guide, my role is to walk alongside you with compassion, presence, and practical support. I offer emotional, spiritual (non-religious ) and logistical guidance—but there are important limits to the services I provide. I do not offer:

 Legal advice or the creation of binding legal documents

Medical advice, diagnosis, or treatment of any kind

Nursing or hands-on medical care (such as administering medications or wound care)

Mental health therapy

Funeral home services or estate planning
